To enhance and augment ongoing safety net integration and quality improvement efforts embarked upon in the County of Monterey to advance the ability of California's healthcare safety net to coordinate care for low-income populations through implementation of new strategies that strengthen the integration of community health centers (CHCs), safety net hospitals and county-operated safety net providers.
